The answer is Primary, secondary, and tertiary structures would be altered. This is due to the fact that an early frameshift mutation will change most amino acid of the polypeptide chain due to a change in the reading frame. This will cause different interaction between the amino acids in the primary protein from those of the normal protein. The formed secondary and tertiary structures will, therefore, also be altered.
