I believe; CAA codes for glutamine, therefore, there is a change in the amino acid sequence. It would not be correct to say now this person has a chance of developing diabetes, because A. the first codon of insulin isn't proline and B. the N-terminus of insulin contains a signal peptide, which is removed from the final, active product.
