If two patients need the same resources in the same medical situation, it is justifiable, when resources are scarce, to limit them to patients having insurance and showing an ability to pay for the resources.

    Healthcare services always try to maintain equity, where there ideally should be "equal resources for equal needs," such that patients suffering from the same condition, should be given equal access to care or treatment regardless of age, gender, race, and ability to pay. This is called horizontal equity. However, patients with more serious or severe forms/complications of the same condition tend to generally be given more care. This is called vertical equity.
