The insurance company will not issue Bob a policy until all their questions have been answered. No company will issue a policy unless they are certain about the health condition of the insured individual.

Since the first physician was not able to answer all the insurer's questions, they will probably request Bob a second visit to another physician without any cost for Bob. The company should pay for this second opinion.
