Metaphors compare a few things at a time.

While extended metaphors compare the same thing over a few lines, pages, or even whole stories.

The way that metaphors and extended metaphors differ is: A metaphor is a limited length, but an extended metaphor is created over the course of a poem. Meaning, a metaphor is when the author uses a comparison between two different subject or objects in the poem. An extended metaphor happens when the author keeps using a metaphor over and over again throughout the poem.
