in the passage, William's injury is described as "shattered bones in his right leg." Part of answering this question for me was process of elimination. The descriptive narration does not describe William and Caroline's relationship. It only identifies her as his wife. We also don't know enough about Caroline from this passage to know if she changed or not. Lastly, the narration does not tell us how he was injured.
