How and why were the cold war geographic boundaries drawn?

When soviet and us/allied forces drove there invasions toward the enemies positions, such as the center of berlin or near the 38th parallel in korea or the 17th parallel in vietnam, thats where the leadership of those particular combatants would draw the line.
