It was commonly believed that association areas had a role in spatial orientation, but recent studies show that other multiple regions of the human brain manage orientation in space.

The most important being the hippocampus, a structure located beneath the cerebral cortex that is a part of the limbic system. Besides the hippocampus, the parietal lobes of the cerebral cortex also play an important role in orientation in space.

So, in this case, Mr. Santore's stroke most likely has affected one of the two structures mentioned above, the hippocampus and/or the parietal lobe of the cerebral cortex.
